You must have all the following when you come in or you will have to reschedule your appointment:

OUTPATIENT TREATMENT

• CALL FOR FEES $
• Picture ID
• Insurance Information
• Any referral papers

ADSAP/PRI

Must have:

• CALL FOR FEES $
• 10 year driving record
• Picture ID
• Court Papers
• Total cost can be $500 to $2400

PARENTING

Must have:

• Picture ID
• $10 book fee

DOMESTIC VIOLENCE/ANGER MANAGEMENT

Must Have:

• CALL FOR FEES $
• Picture ID
• Referral papers

IF YOU ARE MORE THAN 10 MINUTES LATE YOU WILL NEED TO RESCHEDULE YOUR APPOINTMENT
